New Clients

Please complete either the adult or pediatric intake paperwork, based upon whether the client is 18 years of age or older. Please also complete the Billing Intake Paperwork for both pediatric and adult clients.

You will need to bring the completed paperwork to the first appointment or arrive 15 minutes early to complete the two intake packets in the office.

Summit Mental Health Services does not accept insurance and accepts only self-pay, by check/cash or Venmo payment. To schedule your first appointment, please email.
